ABOUT CRL CRL BenefitsWe are pleased to introduce our 2009 employee benefits program for Clinical Reference Laboratory employees and family members. This program offers eligible employees a variety of benefit choices they may select to meet their specific needs. The primary objective is to provide competitive benefits to employees and their covered family members that will help protect them from catastrophic financial loss in the event of illness, injury or death. We also provide a retirement plan for employees who meet the eligibility requirements. Table of Contents
This section is intended to provide you with a general summary of the benefits that are either provided for and/or are available to you as an eligible CRL employee. Detailed descriptions of each program are outlined in the Summary Plan Descriptions. We consider our benefit program to be an essential part of our overall compensation package and hope that the benefits package will help you meet your own individual needs and the needs of your family members. Insurance BenefitsEligibilityYou are eligible to be covered under our Medical, Dental, Life and Section 125 Plan if you are a full time employee. Coverage will be effective when you have enrolled and completed 90 days of full time employment. Each year we have an open enrollment period during which you may review your benefit elections and make changes. Medical PlanThe objective of our Medical Plan is to provide you and your eligible dependents with a competitive and affordable plan. We offer a PPO plan that is underwritten by Humana.
Dental CoverageOur Dental Plan, underwritten by Delta Dental of Kansas, provides comprehensive coverage for diagnostic and preventive services at 100%, basic services at 90%, restorative and orthodontia services at 50%. Orthodontia coverage is available to both dependent children and adults. You and your eligible family members have the choice of selecting any licensed dentist to provide your care. If you select a Delta Dental of Kansas dentist to provide your care, they will file all claims and not charge you above Delta's usual, customary and reasonable fees. Delta will pay the dentist directly, and you are responsible only for your deductible and/or coinsurance (if any) at the time of your visit.
Employee Life InsuranceYou are covered for one times your salary with a minimum of $15,000 and a maximum of $50,000.
Section 125 Flexible Spending PlanOur Section 125 Plan includes several components. The Tax-Saver Plan enables you to utilize pre-tax dollars to make premium contributions for your health and dental coverage. This reduces your contribution amount by federal, state and social security taxes.
Flexible Spending Plan Privacy Notice >> Short Term DisabilityShort Term Disability Insurance protects individuals from uncertain risk through income replacement in the event of a disabling injury or illness. Short Term Disability supports an employee's return to work by providing an income source.
Long Term DisabilityLong Term Disability Insurance provides protection from loss of earnings that you may incur should you become totally disabled and unable to work for more than six months.
Benefits may continue up to age 65 and in some cases longer, should you become disabled after age 60. Voluntary BenefitsOur voluntary benefit program offers life insurance plans, disability income plans, cancer insurance and other programs. The plans are portable and payroll deduction is available for most plans.
401(k) PlanOur 401(k) Plan helps you save for your retirement.
VestingYou are always 100% vested in your own contributions.
Employee BenefitsVacationFull time employees earn 20 hours of vacation for every three months of employment. After five years of service, full time employees earn 30 hours of vacation for every three months of employment. After ten years of service, full time employees earn 40 hours of vacation for every three months of employment. Part time employees with benefits, those who are scheduled to work at least 20 hours per week, will earn 10 hours of vacation after three months of employment. Vacation accrues at the rate of 10 hours for every three months of employment. After five years of service, PTB employees will earn 15 hours of vacation for every three months of employment. After ten years of service, PTB employees will earn 20 hours of vacation for every three months of employment. HolidaysFull time employees are paid for 8 hours of holiday pay and part time employees, scheduled to work at least 20 hours per week, are paid for 4 hours of holiday pay on the following days:
Personal DaysFull time employees accrue 16 hours on January 1 of each year. Part time employees with benefits accrue 8 hours on January 1 of each year. New employees will accrue personal time upon the completion of their first year of employment. Sick PayFull time employees will accrue 48 hours of sick time each full calendar year. Part time employees with benefits will accrue 24 hours of sick time each full calendar year. New employees will accrue a pro-rated amount of sick time based on their hire date. Educational Assistance PlanThe purpose of the Educational Assistance Plan is to encourage eligible employees to enroll in outside educational courses that will help maintain and improve the skills required in the performance of their jobs. The approved degree program or courses must directly relate to the employee's current position.
Employee Assistance Plan (EAP) The EAP is a confidential information, support and referral service designed to help you cope with every day problems that have added stress to your life. This benefit includes a 24/7 telephone help line, face-to-face assessment and referral, short-term intervention, legal and financial services and family resource services. This benefit is available to all employees and their immediate family members.
